This form is valid for all church activities from June 1, 2025 until December 31, 2027.

I permit my child (name and information below) to attend and participate in activities sponsored by FIRST METHODIST LOCKHART. My permission is granted for the leaders of FMC’s children and youth ministry areas to obtain necessary medical attention in case of sickness or injury to my child. I understand that FMC will not be responsible for medical expenses incurred solely on the basis of this authorization. I further agree to notify the leaders in writing of any healthcare changes that would restrict my child’s participation in any normal FMC-sponsored activities. I unconditionally release both FIRST METHODIST CHURCH OF LOCKHART and ALL LEADERS of ALL CLAIMS.
